Give the patient’s name, phone number, symptoms, any information on the toxic agent ingested or inhaled, and time elapsed. Also give patient’s age, weight, allergies, and medications. Have the poisonous agent with you when you call. Staff will make referrals and follow-up calls if necessary. www.mitoxic.org
